PT Licensed Practical Nurse-Wakefield

We are currently seeking a part-time Licensed Practical Nurse for our day program located in Wakefield, MA. The Licensed Practical Nurse will assist the Health Services Coordinator, RN. in providing healthcare services that assist individuals in attaining and maintaining an optimum level of wellness across their lifespan.


20 hours per week, Monday - Friday 10:00 AM - 2:00 PM

  • Provide Medication Administration Management, as needed.

  • Provide Personal Care for participants following clinical team and doctor recommendations/protocols including required nursing procedures (i.e. catheterization.)

  • Provide backup support for Life Choices Programs located in Beverly as needed.

  • Assist in the development and facilitation of training for staff and individuals receiving services.

  • Participate and assist with intake process and orientation of new individuals to Life Choices health care services.


Job Requirements

  • Must hold current Massachusetts LPN license

  • Valid Driver license, acceptable driving record and reliable transportation.

  • Acceptable CORI report and suitable fingerprint results required.

  • Excellent verbal and writing skills. Ability to communicate effectively with diverse population.

  • Flexibility, poise, tact and the ability to develop rapport with individuals of various backgrounds and positions.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
