Hybrid - Clinical APC Manager, ACO Programs - Wakefield/Essex County

Position StatementThe ACO Clinical Manager is responsible for ensuring delivery of primary care and complex care management functions of the ACO teams.
This role is responsible for the successful management of team leads across the organization.
The Manager contributes to programmatic and strategic growth of the ACO program and represents this work internally and with external partners.
OverviewCommonwealth Care Alliance's (CCA) ACO inter-professional teams are responsible for providing primary care and care management to a specific panel of high--risk, Medicaid ACO eligible, non-CCA patients.
This is a group of individuals with significant medical, behavioral, and social complexities that require intensive clinical management.
This cohort of patients belongs to outside organizations that are contracted with CCA to provide this complex care.
The Manager, ACO Clinical role is to ensure the success of the ACO teams and directly manage the ACO Team Leads across the organization.
This role models and develops efficient, highly productive, and effective clinical teams.
The role requires skills in data evaluation, implementation of strategic approaches, and application of current healthcare concepts.
It leverages strong interpersonal skills across the organization and with external partners.
Key Responsibilities Robust understanding of CCA's contractual obligation with each partner organization, as well as key performance indicators that define Identification of strategic team needs to be successful within the contract Direct daily management of the inter-professional ACO clinical team.
This can include 1:
1 meetings with staff, reviews of productivity standards, whether quality of care and documentation standards are met and constant knowledge of key performance indicator status, among other things.
Routine program evaluation and support that includes chart reviews, technical support, logistical needs and clinical Regularly providing individual and team feedback based on data, key performance and quality.
Fair and equal management of every team member, with the goal of success in every role.
May leverage CCAs human resources processes when Facilitates in person and remote inter-professional meetings that focus on clinical performance, operations, strategy, and program Work cross-functionally within the organization to develop and document effective and efficient workflows.
Leverage available data and translates this for inter-operability within the ACO teams.
Partner with, mentor, and manage the other ACO Team Leads across CCA.
This includes but is not limited to 1:
1 meetings, Team Lead performance reviews, feedback, and professional development.
Ability to rapidly adjust strategy to meet team needs.
Partner with senior leadership regarding strategic model approaches, programmatic strategy, and implementation.
Participate in regular meetings with CCA ACO teams, CCA senior leadership.
Provide clinical care to members via telehealth technologies (video, chat, etc.
) for a clinically appropriate clinical care and care management services.
Secondary Responsibilities Participate in cross-organizational working groups to continuously improve practice Participate in CCA Quality Improvement efforts.
Participate in committees and workgroups that promote clinical excellence and help to advance CCA's mission and business objectives.
Actively participate in the evaluation of own performance and progress.
Conducting (as needed) direct patient care which may include:
Primary care to members -Provides patient-centered, comprehensive home-based primary care to complex adult and geriatric patients (assessment, diagnosis, treatment, referrals) to promote continuity of care by coordinating with PCP and other providers as necessary.
Specialty care to members -Provides urgent/sick/episodic care, post-hospital care and palliative care.
Coordinates other specialty services such as behavioral health, psychopharmocotherapy and health education in collaboration with PCPs and CCA clinical specialists as needed.
Minimum Education Required Active NP or PA license Preferred Educational Experience APC with management experience strongly preferred Minimum Experience 5
years of related clinical experience 3
years of management experience.
Significant experience with quality / process improvement work Experience with program management and operational implementation Experience with cross-functional/ inter- professional collaboration Strong management and team-building skills Experience with change management implementation concepts Excellent communication and interpersonal skills Knowledge, Skills and Abilities In-depth knowledge of CCA resources, clinical guidelines, systems, and processes Ability to teach, mentor, and coach across disciplines Ability to quickly shift gears and move in a new direction without clear guidance Familiarity with current healthcare structure, policy, and payment reform models Operational experience of quality improvement and implementation work Willing to learn and utilize telehealth technologies (video, chat, etc.
), when appropriate, for a variety of clinical care and care management services.
Working Conditions Office environment with occasional field visits and frequent travel for meetings across CCAs geography Coverage area:
